It's not a glitch, it's the auto-sub setting.
Go to the main screen, then go down to My Madden ----> Gameplan ---> Auto-Subs.
So, auto-subs work like this. Let's say your RB subs are set to 70 out, and 85 in. This means that once your RB hits 70 fatigue, he will be subbed out until his fatigue regenerates to 85, when he will be subbed back in. If your sub in sliders are set too high so that a player cannot recooperate that amount in a single game, you'll see them leave for the entire game.
Here are the auto-sub sliders I use. They work good:
Out/In
QB: 60/80
RB: 75/82
WR: 74/85
FB/TE: 60/80
OL: 55/75
DT: 74/85
DE: 75/90
LB: 70/80
CB: 60/80
S: 60/80
