It is the laser on your disc drive. It's shot. Very easy to replace, the laser with assembly (so that you don't have to calibrate it yourself) is about $60-70. Pretty easy to install if you are at all mechanically inclined. I did this about a year ago on my launch 60GB.
HOWEVER.....2 months ago I got the yellow light of death (I had no idea WTF this was till then, if you don't, look it up!!). My original PS3 launch console now sits in a box in my storage area. I bought a brand new PS3 slim instead of sending in the original for fixing. They send you back the same launch 60GB, but it won't be "your" machine.
If this new PS3 USB hack device is legit, I'll send the old one in and get it fixed and use that for the USB device.
How are you sure it's the laser? Also, it is not the YLOD either so that's good.
Also, I am able to sometimes get a game of Madden in when I turn the ps3 off from the back, leave it off for a lil bit, then turn it back on. However both times when I went to play another game after finishing the first one it froze after loading for the coin toss.
It is becoming very prominent with the launch consoles in the last year due to age and heat on the board. The "fixes" are just temporary BS, eventually it's going to go tits up once again. The console will freeze/lock up. The light will blink red. Shutting off power and turning it back on, then hitting button on front will cause the green light and fans to come on for a second, followed by a series of beeps, a flash of the yellow light (same area as red/green light), and then it goes back to blinking red. If you have a disc in the drive, you either take the PS3 apart and manually remove it, or you send the unit to Sony to be fixed for $150 and they give you a new game disc. Either way though, all of your game saves, pics, music, video files, are toast. They do not swap them during the fix, and you cannot get it to reboot in any form.
I was lucky, I was able to get it to boot once after I knew WTF was going on. I got my saves and pics onto a USB drive. That was the last time it booted.

If it only happens when you are attempting to, or actually are, running something from a disc....I'm 99% sure it is the laser.
Well, it looks like Backing it up and reformatting might have fixed it.. Been able to play lately no problem..
So reformatting the HDD is going to wipe everything, right?
I always wondered about whether or not having all of this info stored on there and not being able to use tools like disc cleanup or defragging it. Especially with someone like me that has a shit ton of stuff stored on there.
Under system settings there is an option called Backup Utility where I backed everything up onto my external. (Except trophies though and maybe some other stuff).

What's weird is that I didn't think I had any trophy issues when I did my HDD swap but I went to play Linger in Shadow the other day to get the trophies I missed. It showed I didn't have any of them. I wonder how many other games are like this?
When my cousin did his HDD switch none of his Rock Band trophies carried over. Every time he boots up the game it shows 0 trophies unlocked.

Most likely a temp. fix. When my og 60gb w/ BC stopped spinning, I saw people online who said it worked from a couple of months (at most) and it finally died on them. I just went out and bought a 250gb, so far no problems. It actually died on me on my Bday in Feb...so that sucked for me. I would start saving your $ to buy yourself a new one.
