There weren't any punches thrown, just a lot of shoving so I don't know if anyone should have been tossed but usually you see a ref getting in between players and moving them to the sidelines as fast as possible.
The reason that play went on forever is that the officials took 10 minutes to clear the pile and actually announce who had possession of the football and where to spot it. While they were huddled up trying to figure out what the hell was going on guys were just walking around the field (there's a gif of an Atlanta DB just flopping on after walking into a Denver lineman to try and draw a call), both of the sidelines were empty, coaches were yelling at the officials and God only knows what else.
None of that should be going on during a dead ball and I'm am unable to recall a time where it was happening so often.

Oh, and a Denver lineman peeled a ref off the pile. Didn't know you could touch refs now?
